This does not work. It just takes you to the website and there is no place to do anything about the free tube of toothpaste. What a waste of time.

I took the time to fill out this form yet when I went to submit it said I already submitted for this. Well I have YET to get my free sample which I was really anxous to try!

It worked for me until I clicked “submit” – then it kept giving me an error code saying I needed a 6 digit postal code. ?????? 6 digits???? I’m confused!

Ok – I figured out the problem – I had to change countries to be the correct country! But then I went through the whole thing and it said I had already completed the offer. :(
